For children age 6-15
	For children who are age six or older, the SSA Department will use the Children’s Ohio Eligibility determination Instrument (COEDI) substantial functional limitation in at least three of the following areas of major life activity, as appropriate for his age:

	*	Self-care
	*	Receptive and expressive language
	*	Learning
	*	Self-direction
	*	Capacity for independent living



For individuals age 16 and above
	For individuals who are or older, the SSA Department will use the Ohio Eligibility Determination Instrument (OEDI) substantial functional limitation in at least three of the following areas of major life activity, as appropriate for his age
	*	Self-care
	*	Receptive and expressive language
	*	Learning
	*	Mobility
	*	Self-direction
	*	Capacity for independent living
	*	Capacity for economic self-sufficiency
