Eligibilty for Services 



In order to become eligible for services from the Ottawa County Board of Developmental Disabilities, eligibility must be established by the Service and Support Administration (SSA) Department.
 

Developmental Disablity Means:

    A severe, chronic disability that is characterized but the following:  
         
    * It is attibutable to a mental or physical impairment or a combination of mental  
      and physicial impairments, other than a mental or physical impairment solely caused  
      by mental illness  
    * It is manifested before age twenty-two;  
    * It is likely to continue indefinitely  
 

For children under the age of three

 
    Eligibility for Part C early intervention services in the Help Me Grow system includes individuals and the families of individuals in each of the following populations:
 
    Newborns, infants and toddlers who are experiencing a developmental delay in one or more of the following areas:  
    * Cognitive development  
    * Physical development, including vision hearing  
    * Communication development  
    * Social-emotional development  
    * Adaptive development  
      ~ OR~  
    Newborns, infants and toddlers who have a diagnosed physicial or mental condition identified by the Ohio Department of Health - Early Intervention Services, as having a high probability of resulting in a developmental delay.

For ages 3-6

 
    For children who are at least age three but under six must have a least two developmental delays or an established risk;
 
    The SSA Department will review medical records to determine if your child has either a developmental delay; which means;
 
    A child has not reached developmental milestones expected for his chronological age

 
    * Delay shall be demonstrated in one or more of the following developmental areas: adaptive behavior; physical development or maturation (fine and gross motor skills; growth); cognition; communication; social or emotional development; and sensory development; or
 
    * An established risk involving early aberrant development related to diagnosed medical disorders, such as infants and toddlers who are on a ventilator, are adversely affected by drug exposure, or have a diagnosed medical disorder or physical or mental condition known to result in developmental delay such as Down syndrome.
