Infants and Toddlers -Help Me Grow Service Coordinator

Your service coordinator is your key to Early Intervention/Help Me Grow services and your primary contact person! Initially your service coordinator will meet with you to explain the program and explain your rights as a parent. With your permission, your Service Coordinator will help arrange your child's multi-disciplinary evaluation. If your child’s evaluation shows that she or he is eligible for the program, together with your Service Coordinator you will:

Assist you in writing an IFSP plan for your child. An IFSP or Individualized Service Plan is a plan that will identify what your child’s current developmental levels are, what services will be provided to advance those levels, and what goals you would like to see your child reach. It will also contain valuable information about your child’s strength, needs, likes, and dislikes.
Answer any questions you have.
Assist in locating specialized services your child might need.
Help you resolve any problems that might come up, including disagreements about the services you and your child might need.
Link your family to the Help Me Grow Home Visiting Program if you qualify. If the multi-disciplinary evaluation indicates that your child is not eligible for services, your Service Coordinator will connect you with other community services for which your child may be eligible for.
